Why might these medicines affect the brain?
Researchers are still investigating the exact reasons, but several ideas are being studied.
First, better blood sugar control may reduce damage to small blood vessels, including those in the brain. Healthy blood vessels are important because the brain depends on oxygen and nutrients every second.
Second, some diabetes medications may help reduce inflammation. Chronic inflammation is believed to play a role in many age-related diseases, including cognitive decline.
Third, these medications may improve metabolism. The brain uses a lot of energy, and poor metabolic health may affect memory, focus, and long-term brain function.
Finally, weight, heart health, and blood pressure are also connected to dementia risk. Some diabetes medicines can affect these areas, which may indirectly support brain health.
What about semaglutide and drugs like Ozempic?
Semaglutide has received a lot of attention because it is widely used for type 2 diabetes and weight management. Some observational studies have linked semaglutide use with lower dementia risk in people with type 2 diabetes. A Case Western Reserve University study analyzed electronic health records from nearly 1.7 million patients with type 2 diabetes and found that patients prescribed semaglutide had a lower risk of Alzheimer’s disease-related dementia compared with several other anti-diabetic medications. However, the researchers also said randomized clinical trials are needed before making firm conclusions.
This is an important point: observational studies can show a link, but they cannot fully prove that the drug itself caused the lower risk.
Important: this does not mean it is a dementia cure
Even though the research is promising, diabetes drugs should not be presented as a cure for dementia or Alzheimer’s disease.
In 2026, Nature Medicine reported that semaglutide did not slow the clinical progression of Alzheimer’s disease in phase 3 EVOKE trials. This means the drug may still be studied for prevention or risk reduction, but it should not be described as a proven Alzheimer’s treatment.
That difference matters.
A medicine may be linked to lower future risk in some people, but that does not mean it can reverse dementia after it has already developed.
Metformin is also being studied
Metformin, one of the oldest and most commonly used diabetes medicines, is also being researched for brain health. A 2026 study in Communications Medicine compared multiple diabetes therapies and found that metformin showed strong potential in pathways related to diabetes-associated Alzheimer’s disease. The authors emphasized that diabetes therapies may differ in how they affect brain-related biological pathways.
This shows that the story is not about one “miracle drug.” It is about understanding how different diabetes treatments may affect the aging brain in different ways.
What should people do with this information?
